Person centred care and support:
We specialise in supporting people with learning disabilities, sometimes combined with autism and complex behaviours. Our mission is to deliver a high-quality person-centred service that transform lives. We focus on getting to know people, how they would like to be supported, their goals in life and what matters to them most.
With the help of their loved ones and others involved in their care, we build a picture of their likes, routines and what they hope to achieve. Our services are built on the foundation of Active Support and Positive Behaviour Support principles. Our support staff are trained in Positive Behaviour Support, and have skills to support people to live healthy, safe, fulfilling lives, regardless of how complex their needs or challenging their behaviour may be.
How it works:
Following assessments from Adult Social Care and ourselves, discussions are held with you, your loved ones, Adult Social Care Services, and ourselves to decide:
• If you can live with others
• Share with another person or
• need to live alone.
This will determine how many hours of support and the number of support staff you require.
Support Levels:
Some people share support staff.
Some require 1:1 support.
Some people may require 2:1 support, or even 3:1 support.
Getting to know you:
We gather as much information as possible about you and create support plans including any positive behaviour support plans, risk assessments, a communication passport, a health action plan, and a physical activities plan. The information is used to create a person-centred file which has your history of life events, circle of support, likes and dislikes and any other important information.For example:
- the things which are important to you
- your strengths and existing support
- your options for remaining independent
- the additional support you need to meet your needs
An activities schedule with activities that you enjoy and are meaningful to you is designed with you and will help you towards becoming fully involved in the community, for example access to employment, education, /training, and volunteering services.
Giving You the Right Home and Staff Support
You can be involved in the selection of staff who will be supporting you. These staff will be trained to know and understand all your needs. We can support you to find the right home. Once you are happy with your home, on moving in, you become a tenant. If you have someone who helps you with claiming benefits that’s Wonderful! If not, we can help you to:
• Claim housing benefit and all other benefits.
• Manage your food and utility bills.
Promoting Your Independence
Staff will support you with:
• Keeping your home clean and tidy and reporting any maintenance issues.
• Food shopping and cooking healthy meals.
Whether requiring guidance, supervision, or full assistance, you will be supported as required whilst promoting your independence to live in your own home. You will be treated with respect and dignity, and we will promote your human rights and choices.
