Mental Health Support

We support vulnerable adults with enduring mental health illnesses to lead full active independent lives. Referrals come from different sources including from commissioners of mental health services. The people we support require decent accommodation with a good social support system to live safely and successfully in the community.Our support enables individuals achieve their personal ambitions and attain stability in their mental health.

How it works:

Following a referral from a health or social care professional, such as a social worker, community mental health nurse, local authority commissioners or care coordinator, we meet with the person being referred to understand their situation and how best we can help them.

Person Centred Support Plans:

Our Support plans are based on recovery, with several long-term objectives. We review the person-centred support plans monthly. Different people achieve stages of recovery at different paces, and to varying levels, as such our emphasis is always to support individuals to progress at their own pace, with the aim of helping them to live independently in the community.

Support plans include an element relating to risk management and how to manage any crisis that might occur. We act strictly within the guidelines of the Mental Health Act in supporting the various needs of the people we work with. These guidelines ensure we uphold the rights of individuals and assure their safety, as well as that of those around them.

Promoting Independence:

Staff will support people with:
• Keeping their homes clean and tidy and report any maintenance issues.
• Managing activities of daily living such as shopping,self-care needs,
• Managing relationships and interactions and any other support that may be needed.

Whether requiring guidance, supervision or full assistance people are supported as required whilst promoting their independence to live in their own home. Staff will always treat people with respect and dignity, promoting human rights and choices.
